IntelliStaff Medical is actively recruiting Registered Nurse Case Managers for full-time, permanent positions with hospitals across the United States. This opportunity includes TN visa assistance for eligible Canadian and Mexican RNs, and state licensing support for North American nurses. Possible green card sponsorship for qualified nurses will be considered pending a successful performance review at 6 months.
As a direct hire of the hospital, you’ll receive full employee benefits and work alongside a collaborative, mission-driven team to ensure patients receive timely, appropriate, and cost-effective care.
Qualifications of a Case Manager RN
- Active RN license in the specified U.S. state (State licensing assistance available).
- Bachelor’s degree in Nursing (BSN).
- NCLEX required for Canadian and Mexican RNs.
- BLS certification required.
- Minimum of 1 year of recent case management experience preferred.
- Prior experience in care coordination, discharge planning, utilization review, or community health is an asset.
- Strong organizational, assessment, and decision-making skills.
Responsibilities and Duties
- Perform case finding, intake assessments, and development of individualized care plans.
- Coordinate patient care across the continuum to ensure appropriate services and timely discharge.
- Conduct utilization management activities to monitor resource use and medical necessity.
- Identify and address psychosocial and economic barriers to care.
- Evaluate patient outcomes and facilitate safe and effective care transitions.
- Communicate with healthcare providers, insurance representatives, social services, and patients/families to ensure seamless care coordination.
- Close each case with proper documentation, patient education, and follow-up plans.
- Bring a warm, positive, and respectful approach to all interactions—with patients, families, staff, and community partners.
